At Shakthi Vaidya, we are committed to protecting the privacy and security of our patients’ personal health information (PHI). In accordance with the Health Insurance Portability and Accountability Act (HIPAA), we have implemented the following privacy and security policies and procedures to ensure that our patients’ PHI is protected.

Collection and Use of PHI: We collect PHI from our patients for the purpose of providing high-quality medical care. This information includes, but is not limited to, patients’ names, addresses, telephone numbers, email addresses, medical history, and treatment information. This information is used to diagnose and treat our patients, to coordinate care with other healthcare providers, and to bill for services.

HIPAA Compliance: Acupuncture Clinic complies with the HIPAA Privacy Rule and the HIPAA Security Rule. We have implemented appropriate administrative, physical, and technical safeguards to protect the confidentiality, integrity, and availability of our patients’ PHI.

Security Measures: To protect our patients’ PHI, we have implemented the following security measures:

• Limited access to PHI to only those employees who need it to perform their job duties

Breach Notification: In the event of a breach of PHI, we will notify affected individuals in accordance with HIPAA regulations and will take appropriate steps to mitigate the breach and prevent future breaches.

Individual Rights: In accordance with HIPAA, our patients have the right to access and control their PHI. This includes the right to request a copy of their PHI, to request that their PHI be amended, and to receive an accounting of disclosures of their PHI.

Complaint Resolution: If a patient believes that their rights under HIPAA have been violated, they may file a complaint with the Department of Health and Human Services. Our patients may also file a complaint with Acupuncture Clinic by contacting our Privacy Officer.
